Creating a personalized housing transition plan. -
Assisting with benefit applications to secure affordable housing, including eligibility determination. -
Supporting the housing search and application process. -
Guiding the tenant screening and housing assessment process. -
Providing transportation for housing-related discussions. -
Helping with budgeting and financial planning. -
Explaining lease agreements and assisting with negotiations. -
Facilitating connections with potential landlords. -
Supporting cultural considerations with landlords and property managers. -
Assisting in securing funding for deposits. -
Organizing and coordinating the moving process. -
Researching and contacting potential housing options. -
Identifying financial assistance for deposits and home essentials. -
Finding resources to cover moving costs. -
Completing housing applications on behalf of the client. -
Assisting with record expungement or obtaining reasonable accommodations. -
Identifying benefits and services to address housing instability. -
Ensuring the new home is safe and move-in ready. -
Arranging adaptive housing modifications. -
Coordinating assistive technology for housing needs.
A list of not covered services:

Coverage for room and board, including moving expenses. -
Assistance with deposits. -
Provision of essential furnishings. -
Support for food expenses. -
Rent assistance. -
Coverage for utility costs. -
Charges for missed appointments. -
Compensation for staff travel when the client is not present or when housing services are not discussed. -
Delivery of services in a group setting, either directly or indirectly. -
Transition services are not provided if the client passes away before discharge or if services extend beyond 180 days. -
Housing transition services must not duplicate other available assistance. -
Housing tours outside of Minnesota are not covered.
