We offer Housing Stabilization Services for those without a waiver, case management, or an MSHO/MSC+ care coordinator. Our consultants help create a personalized Housing Focused Plan (DHS-7307) and connect you with the right housing provider—ensuring tailored support without duplication of other assistance.

A list of covered services:

• 
  Assisting individuals in obtaining the necessary documentation for Housing Stabilization Services eligibility.
• 
  Creating a Housing Focused Person-Centered Plan tailored to assessment results.
• 
  Helping individuals identify their housing needs, preferences, and cultural considerations.
• 
  Guiding individuals in selecting the right housing transition or sustaining provider.
• 
  Providing resources for services that support non-housing-related personal goals.
• 
  Collaborating with other service providers involved in the individual’s care.
• 
  Educating individuals about their rights, privacy, and appeal options.
• 
  Conducting annual updates to the person-centered housing plan.

A list of not covered services:

• 
  Housing-Focused Person-Centered Plans for individuals with a Medicaid-funded case manager, MSHO/MSC+ care coordinator, or those deemed ineligible for Housing Stabilization Services by DHS.
• 
  Missed appointments.
• 
  Gathering documentation and verification after completing the Housing Focused Person-Centered Plan.
• 
  Conducting assessments.
